From the signature pouts of Hollywood A-listers to everyday beauty lovers, lip fillers have become one of the most sought-after cosmetic treatments. The appeal? A fuller, more defined lip shape that enhances natural beauty. But achieving the perfect pout isn’t just about volume—it’s about structure, balance, and expertise.
Before you book your appointment, it’s essential to understand the different types of lip fillers available. The right filler for you depends on your desired look, lip shape, and long-term goals. With expert guidance from Dr. Michele Koo, a board-certified plastic surgeon, you’ll learn what works best for you.
WHAT ARE LIP FILLERS?
Lip fillers are a non-surgical cosmetic procedure that enhances lip shape, volume, and definition. They work by injecting a gel-like substance into the lips to create a plumper, more youthful appearance. They come in both temporary and more permanent options, but in the world of aesthetics, “forever” isn’t always the best choice.
Most modern fillers use hyaluronic acid, a naturally occurring substance in the skin that provides hydration and plumpness. Temporary fillers allow for flexibility, adjustments, and a natural fade over time, making them a top choice for those new to enhancements.
“I advise my patients to stick to temporary fillers, as they provide more control over your appearance and allow for adjustments as your facial features change over time.”

DIFFERENT TYPES OF LIP FILLERS: WHAT’S RIGHT FOR YOU?
Not all lip fillers are created equal. Let’s explore the most popular options and what they can do for your lips.
HYALURONIC ACID LIP FILLERS – THE GOLD STANDARD
Hyaluronic acid (HA) fillers are the most commonly used and are Dr. Koo’s preferred option due to their safety, natural-looking results, and minimal downtime.
POPULAR BRANDS OF LIP FILLERS:
- Juvederm: Ideal for soft, smooth, and voluminous results.
- Restylane: Provides more structure and definition, great for shaping. Softer fill for movement and very fine lines.
Best for: First-time lip filler patients, those looking for subtle enhancement, and natural-looking volume.
COLLAGEN-BASED LIP FILLERS
Collagen lip fillers were once the go-to option, but they’ve largely been replaced by HA fillers due to better and longer-lasting results.
Best for: By and large outdated and not used. Previously allergies could result and longevity was much too short compared to hyaluronic acid fillers today.
FAT TRANSFER LIP FILLERS – FOR THOSE WHO LOVE A BIT OF EXTRA COMMITMENT
This method involves using your own body fat, usually taken from the thighs or abdomen, to enhance lip volume. However, there are pros and cons:
THE PROS:
- Uses natural body fat.
- Long-lasting results.
THE CONS:
- More expensive than HA fillers.
- Requires minor liposuction.
- Often, fill-in lips can be too “bulky” and lumpy.
- Long-lasting results, which may not be desirable if shape/size is incorrect.
- Not malleable.
People looking for a more permanent yet natural-looking solution tend to explore this option.
SILICONE & OTHER MORE PERMANENT OPTIONS – PROCEED WITH CAUTION
Polytetrafluoroethylene (PTFE) tubes have been used in the past for lip augmentation as permanent implants. However, they are now rarely used today due to newer advancements of hyaluronic acid and fat grafting, which are safer. Silkon 1000 is an off-label use for permanent lip augmentation. It is not FDA approved for that purpose but is often used by plastic surgeons and dermatologists for “off label” lip augmentation.
Dr. Koo advises against these in favor of temporary options for the first lip injection. Once you are comfortable with the size, shape, and look of your lips with a temporary fill, then the permanent filler of Silkon 1000 is wonderful if injected by skilled, experienced injectors and physicians.
“If you’re considering more permanent options, always start first with a temporary filler. Proceed with caution and only after you discuss ALL of the options available to you with your favorite plastic surgeon. Baby steps is always the way to go for the best results.”

- Technique matters more than the product.
- Clear communication with your provider is essential.
- Dr. Koo’s philosophy: Enhance, don’t inflate.

WHAT TO EXPECT BEFORE, DURING & AFTER YOUR LIP FILLER APPOINTMENT
Getting lip fillers is a process, and, regardless of the type of filler, knowing what to expect can ease any nerves. Here are some pre-appointment considerations to ensure a successful injection:
PREPPING FOR YOUR LIP FILLERS – THE DO’S & DON’TS
To ensure the best results:
- Stay hydrated. Proper hydration can improve skin elasticity, aiding the healing process.
- Limit alcohol and caffeine intake. Reducing caffeine and alcohol consumption a few days before your appointment can help minimize swelling.
- Avoid blood-thinning medication. Reduce the risk of bruising by avoiding blood thinners such as aspirin and ibuprofen.
THE PROCEDURE – STEP BY STEP, KOO BY KOO
STEP 1 – CONSULTATION
Discuss your goals, desired look, and any concerns with an experienced injector. This is also an opportunity to explore different types of lip fillers. Bring photographs to the consultation appointment.
STEP 2 – NUMBING
A topical anesthetic cream is applied to the lips, taking 15-20 minutes to take effect.
STEP 3 – INJECTION & SCULPTING
Your chosen filler is injected, and your injector sculpts your lips to ensure an even and natural look.
STEP 4 – FINAL TOUCHES
Your injector assesses symmetry and balance, making any necessary refinements.
HEALING & LOOKING AFTER YOUR LIP FILLERS
- Apply ice to reduce swelling.
- Avoid strenuous exercise for 48 hours.
- Stay hydrated for optimal results.
